Make a Difference in End-of-Life Care

We are seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our hospice team in providing dignified and supportive care to patients in their final stages of life. As a Hospice CHHA, you will play a vital role in creating a comfortable and peaceful environment for our patients and their families during this sensitive time.

Responsibilities:

• Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and feeding
• Provide emotional support and companionship to patients and families
• Help maintain a clean and safe home environment through light housekeeping tasks
• Monitor patients’ vital signs and report any changes to the nursing team
• Assist with errands and transportation as needed
• Document care provided in accordance with agency policies

Qualifications:

• Current certification as a Home Health Aide (CHHA)
• Experience working with hospice patients preferred
• Excellent communication and interpersonal skills
• Compassionate and empathetic nature
• Strong ability to work independently and as part of a team
• Reliable transportation and a valid driver’s license
