About the Role

As a Home Hospice LVN, you will play a vital role in our interdisciplinary team, providing skilled nursing care and emotional support to patients facing terminal illness in the comfort of their own homes. You will work closely with the patient’s family and other healthcare professionals to ensure physical, emotional, and spiritual well-being throughout the end-of-life journey.

Responsibilities:

• Perform skilled nursing assessments and interventions, such as monitoring vital signs, administering medications, managing pain, and providing wound care.
• Collaborate with the Registered Nurse to develop and implement individualized care plans.
• Educate and support patients and families about hospice care and the dying process.
• Provide emotional support and companionship to patients and families, addressing their anxieties and fears.
• Maintain accurate and timely documentation in the electronic medical record.
• Communicate effectively with the healthcare team, including physicians, social workers, and chaplains.
• Travel to patients’ homes within a designated geographical area.

Qualifications:

• Current California LVN license in good standing.
• A minimum of one year of experience in home health or hospice care is preferred.
• Strong clinical skills and assessment abilities.
• Excellent communication and interpersonal skills.
• Compassionate and empathetic nature with a commitment to providing holistic care.
• Ability to work independently and as part of a team.
• Reliable transportation and a valid driver’s license.
