Home Hospice Registered Nurse

We are seeking a compassionate and experienced registered nurse to join our dedicated team in providing comfort and quality care to terminally ill patients in the comfort of their own homes.

The Role:

As a Home Hospice Registered Nurse, you will play a critical role in our interdisciplinary team, working closely with patients, families, physicians, and other healthcare professionals to deliver compassionate and comprehensive end-of-life care. Your responsibilities will include:

  • Clinical assessment and care:
    • Conducting initial and ongoing assessments of patients’ physical, emotional, and spiritual needs.
    • Developing and implementing individualized care plans in collaboration with the patient, family, and physician.
    • Administering medications and providing other skilled nursing interventions.
    • Managing pain and other symptoms to ensure patient comfort.
    • Monitoring vital signs and assessing changes in patient’s condition.
    • Documenting care meticulously and accurately.
  • Emotional and spiritual support:
    • Providing emotional support and guidance to patients and their families facing end-of-life challenges.
    • Actively listening and communicating with empathy and compassion.
    • Identifying and addressing psychosocial and spiritual concerns.
    • Supporting families through bereavement.
  • Collaboration and communication:
    • Maintaining close communication and collaboration with the hospice team, including physicians, social workers, chaplains, and home health aides.
    • Educating and empowering families to provide care for their loved ones.
    • Liaising with community resources to coordinate additional support services.
  • Professionalism and teamwork:
    • Maintaining a high standard of professional conduct and ethical practice.
    • Demonstrating strong teamwork skills and contributing positively to the hospice team environment.
    • Continuously seeking to learn and improve one’s skills and knowledge of hospice care.

Qualifications:

  • Current RN license in good standing in the state of California.
  • Minimum of 5 years of experience in direct patient care, preferably in hospice or palliative care.
  • Proven ability to manage pain and other symptoms effectively.
  • Excellent communication and interpersonal skills with patients, families, and healthcare professionals.
  • Strong emotional intelligence and ability to navigate sensitive end-of-life situations.
  • Excellent organizational and time management skills.
  • Ability to work independently and as part of a team.
  • Valid driver’s license and reliable transportation.
